2010 NEYM Sessions Registration Form, including the Equalization Request Form for Financial Aid

Youth Program Health/Permission Form. This completed form is required for your child to participate in any Youth Program (including Young Friends who are 18 years old). Submit this form to the Registrar. Submit this form vial email to the Registrar. Download the Form by clicking the PDF icon above. Follow the instructions to fill it in and submit the Form via email to the NEYM Registrar. You may continue to send your child's filled in electronic Health/Permission form via email to the NEYM Registrar through NEYM Sessions. Do not send the form by US Postal mail after July 25th. For other questions or concerns, contact the appropriate Youth Programs Coordinator

Food Allergies and Severe Dietary Restrictions Form. On Saturday, 8/7/2010, at 4pm, a Sodexho Food Service representative will meet with Friends, in the Salmanson Dinning Hall, who have food allergies and/or severe dietary restrictions. The Food Service will work with you to best meet your food needs during your stay at the College. Please bring this form with you to Yearly Meeting.
